About: The U.S. healthcare system struggles with financial limitations and inflexible delivery systems. Mismatches between patient need and provider capability are extensive, and are often resolved expensively in emergency departments or by admission to the hospital. We propose a new strategy – Mobile Integrated Healthcare Practice (MIHP) – to deliver integrated, interprofessional care to patients in their homes. MIHP is a strategy that redesigns the current mobile health delivery framework by forging local partnerships among payers, providers, EMS systems, patients and other community stakeholders.
